"Elevator" covers two genuinely different instruments. A conventional elevator is a lever: a robust blade seated between root and alveolar bone and rotated to mobilise the tooth. A luxating elevator (luxator) is closer to a periotome: a thinner, sharper blade advanced apically around the root to sever the periodontal ligament and expand the socket before delivery. Confusing the two — or using one with the other's technique — is how fine tips get snapped.
Blade geometry tells you the job
- Conventional elevators (Coupland, Cryer, Warwick James patterns) carry thicker, stronger blades that tolerate controlled rotational force. Coupland patterns in graded sizes work as wedges; Cryer triangular patterns engage inter-radicular bone for root removal.
- Luxating elevators have slim, sharp, often slightly curved blades — typically 2–5 mm widths — designed for cutting and wedging along the ligament space, not for levering. Many carry colour-coded handles by width.
- Down-angled and contra-angle patterns exist in both families for posterior access where a straight shank cannot follow the root line.
How the choice plays out at the chair
Most extraction set-ups benefit from both: luxation first to loosen attachment atraumatically and preserve socket walls (worth caring about wherever implant placement may follow), then either forceps delivery or a conventional elevator where mechanical advantage is needed. What to look for when buying
Tip integrity is everything on luxators — inspect the blade edge under magnification if you can; a fine blade with grind marks or asymmetry will not track the ligament cleanly. On conventional patterns, check the blade-to-handle junction: the instrument takes rotational load there, so a one-piece forged shank outlasts a welded one.
