Sinus lift osteotomes: straight vs offset, concave vs convex

Osteotome sets photograph as ten near-identical steel rods. The differences that matter are all at the last few millimetres — tip geometry and shaft angle — and choosing between sets means reading those details, not counting pieces.

1. Straight and offset shafts

Straight shafts transmit force in line — ideal for anterior sites with open access. Posterior maxilla is where the offset earns its place, stepping the working end around the buccal corridor. Serious sets carry both: the ten-piece straight-and-offset concave set pairs each diameter in both configurations.

2. Concave or convex tips

Concave (cupped) tips capture and carry — they advance bone ahead of the instrument, the classic Summers approach for lifting the membrane with a graft cushion. Convex tips compact laterally as they advance, condensing softer bone to improve primary stability. The ten-piece convex set is the condensing counterpart to the concave set above.

3. Graduation is the technique

Osteotome work is sequential: each diameter expands the osteotomy a controlled step. A set with even graduation and clear size identity is a safety feature, not a luxury.
